Transforming Care Programme Coordinator

I am a Transforming Care Programme Coordinator in the Shropshire, Telford and Wrekin team, supporting delivery of the Transforming Care Programme led by NHS England. My work focuses on monitoring key performance indicators for people with learning disabilities and autistic people admitted to mental health hospitals, managing the Dynamic Support Register, and producing performance reports for the Integrated Care Board and NHS England. I also support Care and Treatment Reviews (CTR) alongside clinical teams and contribute to service development projects, including system implementation, digital transformation of the Dynamic Support Register, and staff training. My role combines operational coordination, reporting, and project support to help improve community outcomes and reduce unnecessary hospital admissions.
